 This web site, which was developed with funding from Children's Medical Services (Florida Title V CSHCN Program), includes videos, tips and strategies to help youth and young adults communicate more effectively with health care providers.

 "When You're 18 - A Health Care Transition Guide for Young Adults" is a product of the Health Care Transition Initiative of the Institute for Child Health Policy (ICHP) at the University of Florida.

 This 18 minute video teaches teens how communicate more effectively with their health care providers – share information and get their questions answered.
