The Health Care Transition Initiative
at the University of Florida
The mission of the Health Care Transition Initiative
at the University of Florida is to increase awareness of, gain
knowledge about, and promote cooperative efforts to improve
the process transitioning from child-centered (pediatric) to
adult oriented health care. Our vision is to improve the transition
process for all adolescents and young adults, although our current
efforts focus on those with disabilities and special health
care needs.
Activities carried out through the Transition
Initiative include research, product development, and networking.

The Transition Initiative is a multidisciplinary

Robert W. Gibson PhD, Randy
Miller and Narayan Raum.
Future Activities:
The transition Initiative is interested in developing new partnerships
and working relationships with individuals and agencies that

Our research seeks to gain a better understanding of the process
of health care transition and its relationship to other aspects

Products:
Our products, which include research reports, workbooks, informational
booklets and videos, and on-line training programs, are designed
to enhance the health care transition related knowledge and
skills of adolescents, young adult, families and health care
professionals.
Networking:
Our networking efforts involve hosting an international e-mail
discussion group; and maintaining an informational web site.
