Each of these links is explained below.

Health Care Transitions listserv
Click below to become a member of the Health Care Transition Listserv. Over 1000 youths/young adults with special health care needs, family members and health care providers policy makers, insurers, and others with knowledge and experience in the transition from child-centered (pediatric) to adult-oriented health care currently participate on the listserv. The listserv provides an opportunity to ask questions, discuss health care transition topics, facilitate networking and the exchange of ideas with a variety of interested individuals.

This listserv is a service of two projects of the Institute for Child Health Policy that focus on the transition of youth with special health needs: the Center for Policy and Partnerships Healthy and Ready to Work initiative, which is funded by the federal Maternal and Child Health Bureau; and the Consortium for Children and Youth with Disabilities and Special Health Care Needs' Promising Practices in Health Care Transition initiative, which is funded by the National Institute for Disability and Rehabilitation Research.

Member Profiles Database
Click below to create or edit your member profile. Member profiles are quick and easy ways to identify your interests, experiences and areas of expertise in health care transition and share this information with others. Knowing others who have had similar experiences or have faced similar questions often creates a very strong bond. The member database supports communication between list members by providing a way to share this information. Most of the information requested is optional. However, we hope that you will complete the entire form. Information in the database will only be available to members of the listserv. Member profiles serve as the foundation for the member search function described below. 

What if I am under 18 years old?
Transition is about you, and we want you to be active participants in the Transition Listserv and member database. We only ask that you complete the parental permission section of the form. Upon submission of the form, an e-mail message will be sent to your parents or guardians informing them of your participation in the database.

Finding Information in the Member Database
Click below to search member profiles for a variety of search categories such as age, location or disability experience. Searches can help you identify list member who have interests or experiences similar to your own and facilitate contacting them.
