"Talking With Your Doctor, Nurse and Other Health Care Professionals" is a product of the Health Care Transition Initiative of the Institute for Child Health Policy (ICHP) at the University of Florida and was developed under a contract (COQMQ) from Florida's Children's Medical Services Program (Florida Department of Health) to ICHP.
The GLADD approach to talking with doctors is based on the HealthPACT™ program, which was developed at the University of Colorado, School Health Programs, Denver, Colorado. Our thanks to Judith B. Igoe, RN, MS, FAAN for sharing information about her HealthPACT™ Program.
