Health Care Transition: An On-Line Course for Children's Medical Services (CMS) Professionals
This online training course was developed by the staff of the Health Care Transition Initiative at the University of Florida through a contract from Florida’s Children’s Medical Services (CMS) Program. This on-line course provides CMS Care Coordinators and other CMS staff with an understanding of how and why to transition CMS enrollees with disabilities and special health care needs from pediatric care to adult oriented care. The course addresses legal, social and developmental aspects of health care transition and suggests ways to facilitate and help youth and families plan for transition.

The course has three main objectives:

1. The first object is to provide information regarding health care transition to CMS Care Coordinators to aid them in helping adolescent enrollees and their families plan for and successfully transition to adult oriented health care services.

2. The second objective is to provide specific tools that CMS Care Coordinators can use in planning for and documenting health care transition activities with CMS enrollees and family members.

3. The third objective is to help assure CMS Care Coordinators successfully address the CMS Performance Measures for health care transition.

The course is composed of narrated power point presentations, readings and additional resources, learning activities, online discussion forums and quizzes. CMS Care Coordinators who successfully complete the course are awarded continuing education credits from CMS.

This course is specifically designed to meet the needs of staff of Florida’s CMS Program and access is restricted. However, much of the course content is applicable to professionals serving children in other states and in other settings.
